Developing care plan :Creating individualized plans to achieve rehabilitation goals. Medication management : Administering and monitoring medications. Wound care : Managing wounds, incisions, or pressure sores. Pain management: Implementing strategies to reduce pain. Mobility and ambulation : Assisting patients with movement and mobility. ADL support : Helping patients with daily living activities (e.g., bathing, dressing and oral hygiene) Therapy facilitation : Assisting patients with physical, occupational, and speech therapies. Education : Teaching patients and families about conditions, treatments, and self-care. Collaboration : Working with interdisciplinary teams to achieve rehabilitation goals. Progress monitoring : Regularly assessing patients' progress and adjusting care plans. Discharge planning : Preparing patients for a smooth transition home or to another facility. Family support : Providing emotional support and education to families. Maintaining a safe environment : Ensuring a safe and therapeutic environment. Documentation : Accurately documenting patient information and progress.
